"Seema hurt her elbow" refers to the past, and is thus the normal form.
"Seema hurts her elbow" refers to the present, to this very moment, and could only be used if you were with Seema and happened to be watching her her as she hurts her elbow. One minute later you should say "Seema hurt her elbow" or "Seema has hurt her elbow". Both of those forms are correct.

Both can be correct. Seema hurts her elbow (all the time, every day, sometimes).
Seema hurt her elbow (yesterday, last week, in July).

hurt without the s. s would make the phrase current, as in immediately. example: Seema jumps onto the wall bars and hurts her elbow. Seema jumped onto the wall bars and hurt her elbow.

Their both grammatically correct but it is hard to imagine a situation where one would say the second sentence without additional words of explanation.

If the injury already occurred, the first is correct. If the injury is currently happening, the second is correct.
