Controversial questions, in general, aren't a violation. However, questions are to be knowledge seeking, not rants, statements, baiting, or insulting.

Depends. Would said controversial questions that may be true and offensive also happen to be racist and violent crap? If so, then no.

A real question BY DEFINITION cannot be "true".
As to "offensive"..what's the first thing you read in the guidelines ? BE COURTEOUS . So yes, being offensive on purpose IS a violation

Yes, but what you find to be"true" and "offensive" may be VERY different from other people. If you are intentionally asking provoking bait and phish questions to stir up controversey and inflame the YA community, that is a Terms of Service violation of the YA community guidelines. "Truth" is VERY subjective. You are in a VERY gray area. Of possible harassment and provocation. Your opinion is NOT a fact or always "true." Sarcasm and irony are not obvious in print because there is no tone of voice or body language. YA is NOT a chat forum or a gossip column or personal email. Verizon has disabled personal email.

You can ask controversial questions if you are really seeking knowlege (many arent) and they do not have to be worded offensively. Nothing has to be insulting or offensive.

It depends on how you define "offensive" and "true". It could go either way. The problem is most such questions rise to insulting and are usually rants, not questions.
