How is it possible for Musk to hold citizenship in 3 countries ?

He met the requirements in 3 countries. One by having parents from that country, so that wasn't hard. The next 2 were more difficult and had a lot to do with his advanced STEM degrees, and proven entrepreneurial skills.

The nations he's spent time in all allow for multiple citizenship and he's managed to naturalize in two of them. Turns out when you're a PhD in a STEM subject you can do a lot of things the average person cannot.

There are various possibilities for someone to hold multiple citizenships. E.g. mother holds citizenship in one country, and father in another. E.g. born in one of the countries which allow jus solis - citizenship by birth within the country - but the child will also hold birthright citizenship in its parents' country/ies of citizenship. Another possibility is immigration.

Why ever not? I have 4 citizenships but only carry three passports because the fourth country (South Africa) is not one I wish to be associated with.

There are many countries that allow multiple nationality.

Because he's a South African (1) native, got Canadian citizenship (2) through his Canadian mother, and then migrated legally to the US (3).
Actually, he got his Canadian passport (and moved to Canada) because he figured it would be easier to get into the States as a Canadian citizen. And of course, he was right.
