Is this usually interpreted as "shared infrastructure and dedicated infrastructure", or as "shared, dedicated infrastructure"?

    shared infrastructure and dedicated infrastructure implies (indicates) that a thing could be one, or the other, and yet still be part of the intended group (could be both but being both is not required).  shared and dedicated means both at the same time is necessary (otherwise you would say shared infrastructure and dedicated infrastructure, and you did not).  I presume that is what you take "shared, dedicated infrastructure" to mean (both are true always).
