Can a mentally ill person have a job for 30 years and only have one topic in life that's delusional?

So everything about them is normal except for one topic and if you discuss it with them of course they start talking insane

of course a narcissist can hide it

Update 2:

I'm talking about a co-dependent lady next door it is very weak

Not really, although there are some people who are psychopaths who are very good at "fitting in" to the rest of society and will not murder you for cutting them off, for example, but they might still do things that will get them in trouble like slacking off at work, or having an affair, etc, but they'll use their charm and social skills to get out of it.

Also, there's people with schizophrenia who are normal when on medication, but if they stop taking it sound crazy.

    Sounds pretty unlikely.
