Why would an alcoholic need to see a psychiatrist?

I'm wondering as I have been to a treatment center twice and went right back into drinking again both times... I feel extremely hopeless...

Because often there can be underlying emotional or mental issues that also need treatment. The psychiatrist can refer the recovering alcoholic to the best treatment and followup. It always takes more than just taking the alcohol away to successfully recover. You need to call Alcoholics Anonymous asap. They have tons of free resources for you that can work.
