Is being a transgender ***** in Islam?

Iet's say for example a man who was born gay. and being gay means that he is a female from inside. so can't he just be a trans?

Varies from flavor of Islam to flavor of Islam.

For example: in Iran convicted homosexuals are ***pressured*** (by the theocratic Islamic government) to undergo sex-change surgery.

In some other nations the official "flavor" of Islam disallows sex-change surgery.
