What is the difference between managed and unmanaged dedicated server hosting?

Managed : Server provider will take care of OS installation, security patches, backups etc.
Unmanaged: You just get a server and you have to install, manage everything from OS to everything.

On a managed dedicated server plan, the hosting company can decide to replace all components and take care of the entire infrastructure. However, on an unmanaged dedicated server, the web host is no longer responsible with maintaining the control panel, nor for security vulnerabilities installed by the client.

you need to ask the provider, but typically they offer more options, such as backups, multiple servers for speed/fallback, and other such niceties.

This varies by web hosting company. Read their documentation to see what services that includes.
