Yes. If they possess the altitude. The intelligence to understand, to apply what they learn. Medicine is the ability to recollect and retain facts learnt.
Once upon a time barbers performed operations! Strange but true!

Which of them doesn't have the degree: the person teaching, or the person being taught?
Your question is ambiguous.

No. If someone who is smart enough to become a great heart surgeon, then he/she is smart enough to get a medical degree.

Why? You'd never get a license without the necessary credentials. But, yes, in theory they could. They would still need all of the education that a degree would provide, but it is possible that someone could be taught all that a medical student and surgeon needs to learn without degrees being conferred, but that would be in some post apocalypse situation.

no because it is far more than just technical expertise, you really have to understand physiology down to the molecule as these surgeons also take care of the patient in the ICU, publish papers, give lectures etc. While the "degree" is really a piece of paper, its all the classes required for that degree that are necessary.

Possible? No. With no Medical degree, this "great heart surgeon" could never prove their skill because it is illegal to do that sort of thing without having a MD.

I'm sure it would possible to gain the necessary skills, but without the degree nobody would ever let you practice medicine.
